This is an old revision of the document!
The PMH should contain the following data, in aa easily visible approach:
- name, DOB, insurance number, address, insurance
- CAVE-entries (allergies/intolerances, etc.)
- permanent diagnoses
- current active diagnoses (?)
- permanent medication (→ prescription plan)
- History: course (all of SOAP?)
