The PMH should contain the following data, in aa easily visible approach:

• name, DOB, insurance number, address, insurance
• CAVE-entries (allergies/intolerances, etc.)
• permanent diagnoses
• current active diagnoses (?)
• permanent medication (→ prescription plan)
• History: course (all of SOAP?)
