The PMH should contain the following data, in aa easily visible approach:

  * name, DOB, insurance number, address, insurance
  * CAVE-entries (allergies/intolerances, etc.)
  * permanent diagnoses
  * current active diagnoses (?)
  * permanent medication (-> prescription plan)
  * History: course (all of SOAP?)
